Monument Healthcare Canyon Rim, under the ownership of Gunnison Valley Hospital, was reported in the first quarter of 2026 to be based in Salt Lake County, according to the Centers for Medicare and Medicaid Services (CMS). Gunnison Valley Hospital has held ownership of the nursing home since July 1, 2020.
The facility operates as a for-profit nursing home.
For that timeframe, Monument Healthcare Canyon Rim earned an overall CMS rating of three, falling below the statewide nursing home average of 3.4 in Utah. These CMS ratings are based on multiple criteria including health inspection results, staffing information, and resident care quality.
The data indicate that the facility cared for an average of 64.4 residents per day in Q1 2026 and maintained 90 available beds. The site is not classified as hospital-based.
Financial records from the first quarter of 2026 show Monument Healthcare Canyon Rim faced one fine totaling $6,676 and received one penalty.
Salt Lake County included a total of 25 for-profit nursing homes at that time.
U.S. Census Bureau estimates reflect continued growth in the nation’s older population while younger groups decrease. Between 2020 and 2024, Americans age 65 and older grew by 13%, whereas the under-18 segment declined by 1.7%. Over the previous century, the older adult population has expanded significantly, rising from 4.9 million in 1920 to over 61 million in 2024.
A 2021 study found that almost half of this demographic, about 28 million people, might require long-term services and supports. Furthermore, the study suggests that more than one-third may ultimately need nursing home care.
“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance, an insurance education organization. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
